Work Procedures:
Receiving New Cases:
1. The patient or his/her legal guardian completes the Application to Utilize Program Services.
2. The Program Director reviews the Application to verify that it meets the set conditions and directs the Social Research Team to conduct a comprehensive filed social survey of the case.
3. The Social Research Team visits the affected family to view its social condition and provide advice to the patient relevant to completing the required documentation of the case. The Team completes the Social Study Form including the Team recommendations.
4. The file will not be considered complete unless all required documentation is in file.
5. The completed file is then referred to the respective |Health Committee to determine the priority of the case from a medical point of view and to make recommendations.
6. The file is then forwarded to the Case Study Committee which classifies eligible cases in order of priority based on medical urgency, the health condition, the living condition, and the Program general financial situation.
7. Based on determined priorities, the Case Study Committee makes a decision to accept or reject the case and provide its justification of the decision.
8. The accepted case is submitted for final approval by the Life Makers Administrative Council.
9. The electronic data base is updated by entering the new accepted case and initiating procedures to refer the case to one of the designated medical facilities.
Providing Medical Treatment:
1. The cases are referred to the designated medical facilities for medical treatment in order of established priority.
2. The Program covers all expenses inside the medical facility resulting from the medical treatment, including residence, surgical procedures, lab and x-ray tests, and medical disposables.
3. The program provides transportation expenses to and from the medical facility to patients with complete or partial disabilities preventing them from using public transport and requiring them to use taxi services.
Designated Medical Facilities:
The Life \Makers Society plans to expand its medical facilities network by signing agreements with a number of hospitals to provide medical services for the Program clients and to invoice Life Makers for the expenses.
